What Is The Admission Procedure?
Anyone can make the initial request for services by calling (409) 384-5995 or (800) 661-9047. You can also fill out our online contact form. Contact may be made by the patient, a family member or a physician. The family is encouraged to discuss the issue of hospice care with the patient’s physician.Certain Patients Need Inpatient Care
While hospice care is provided primarily in the home, contract inpatient facilities are available to supplement home care, if needed. Hospice maintains close working relationships with several area hospitals and nursing facilities including those in outlying areas. Certain medical criteria are considered when inpatient care is recommended as an appropriate patient care alternative – for example, pain or other physical symptoms that cannot be managed in the home setting.
